As part of KCEOC, CAP Inc’s ongoing Community Needs Assessment, we are asking that you assist us by completing the following survey. Please rate each item from one to five by clicking of the corresponding one radio buttons, with one representing the least amount of need or problem and five representing the greatest amount of need or problem.
